A contractor was dispatched to a tower to address a worn-down hanger plate, a critical safety concern. The crew reported that they had "replaced" the rusted hanger plate but upon review by the Work Verification team, it was discovered the crew had merely spray-painted the worn anchor.
The hanger plate, a crucial component for safety, was not replaced, raising the risk of potential failure that could lead to an ignition. Recognizing the gravity of this situation, we identified other similar locations of this potential occurrence and reviewed past documentation to ensure this was an isolated incident.
The foreman responsible for falsely reporting the hanger replacement was promptly fired and the hanger plate was correctly replaced, allowing our client to maintain their standards of integrity and safety in field work.
