Contractors reported the installation of fiberglass insulators on wood pole guy wires as a success. But they overlooked a crucial step: the fire-prevention bonding process, putting every asset the crew worked on at risk.
When we discovered the oversight, we immediately contacted the foreman about their work methods and learned many crews had neglected to bond the attachment hardware. For every instance where bonding was overlooked, we requested crews revisit the site and properly bond the hardware. We also provided feedback to field supervisors to educate crews on safe installation practices.
By establishing clear guidelines and fostering a culture of accountability, we ensured that every step of the installation process was safely completed, ultimately minimizing the risk of pole fires.
